Blepharoplasty Surgery (Blepharoplasty)
 
Blepharoplasty can be both a functional and cosmetic surgical procedure intended to reshape the upper eyelid or lower eyelid by the removal or repositioning of excess tissue as well as by reinforcement of surrounding muscles and tendons. When an advanced amount of upper eyelid skin is present, the skin may protrude over the eyelashes and causing a loss of peripheral vision. The outer and upper parts of the visual field are most commonly affected and the condition may cause difficulty with activities such as driving or reading. In this circumstance, upper eyelid blepharoplasty is performed to improve peripheral vision. Patients with a less severe amount of excess skin may have a similar procedure performed for cosmetic reasons. Lower eyelid blepharoplasty is almost always done for cosmetic reasons, to improve puffy lower eyelid "bags" and reduce the wrinkling of skin.

Blepharoplasty is performed through external incisions made along the natural skin lines of the eyelids, such as the creases of the upper lids and below the lashes of the lower lids, or from the inside surface of the lower eyelid. Initial swelling and bruising take one to two weeks to resolve but at least several months are needed until the final result becomes stable. Depending upon the scope of the procedure, the operation takes one to three hours to complete.

Facial aging around the eyes starts to develop early in the mid- to late thirties with excess skin, crow's feet, and lower eyelid bags for some. It only worsens through the 40s and 50s. Yes, there is some noticeable swelling and bruising around the eyes but remarkably, there is usually no pain. You look 'beat up' but you don't feel so. Recovery from blepharoplasty is what I call social recovery. When you feel comfortable again going out in public is what makes up blepharoplasty recovery.

For some patients, they are not concerned about their appearance and they are out in public again in 3 to 5 days. Most patients do care, however, so their full return to the public eye may be closer to 7 to 10 days. Blepharoplasty is one plastic surgery where what you do before and after the procedure can make a big difference in the amount of bruising and swelling that occurs. Stopping any aspirin and non-steroidal medications 3 weeks before surgery is the first step. Taking arnica tablets beginning one week before and after surgery is the next step. Keep your head and your eyes iced for the first 24 hours after surgery. I also place an Actipatch Eye Recovery mask immediately after surgery and onto this is where you place the ice. ActiPatch emits pulsed electromagnetic waves which help reduce swelling on the traumatized eyelids. After the first day after surgery, your main efforts are over other than keeping your eyelids moist with a light coating of antibiotic ointment. (this keeps the sutures soft so that they are easier to remove)

In 10 - 14 days, you will look 'non-operated', even though there is still some small amount of swelling and perhaps some line of bruising along the lower eyelid. By 3 weeks after surgery, you will be able to enjoy the full benefits of your blepharoplasty surgery! You will looke more refreshed and that will undoubtably be noticed...even though most will never figure that it was due to eyelid surgery. (unless you told them!)

What Is Blepharoplasty?
Blepharoplasty refers to any surgery which is used to correct a cosmetic deformity of the area around the eye. It may be used to remove excess skin of the upper eyelid which can result in vision improvement or simply remove the bags around the lower eye. Blepharoplasty can be performed on either the upper or lower eyelids or both at the same time and either the left or the right side or both at the same time.

Indications for upper eyelid lepharoplasty are several. Excess upper eyelid skin can produce a cosmetic deformity or can sometimes interfere with vision. To determine if excess upper eyelid skin interferes with vision, a test is done which measures a person's ability to see objects in their peripheral vision in a natural position and with the excess skin taped out of the way. Excess upper eyelid skin can also be causing a cosmetic problem and for women this often leaves them without a platform to put makeup on. Upper eyelid blepharoplasty involves removing the excess skin of the upper eyelid, possibly with some muscle and closing the incision in a natural skin crease. Once healed, the incision is imperceptible. During the procedure, fat may be removed from the middle part of the eyelid to produce a less full look. Care must be taken as sometimes a low brow may be causing excess upper eyelid skin and the correct procedure to fix excess upper eyelid skin is actually putting the brow back into its correct position.

Lower eyelid blepharoplasty is usually cosmetic procedure. It involves removing the fat from the around the lower portion of the ye which generally causes bulges around the eyes as we age. The procedure can be done from inside of the eyelid or possibly through a small incision in the skin just below the eyelash line. In either case, once the incision heals, it too is imperceptible.

There are risks associated with blepharoplasty procedures and it is always important to seek a trained and experienced surgeon for any surgery. An experienced surgeon can sit and talk with you about the risks and benefits of the procedure. It is important to share with your surgeon all of your medical problems and any medications that you may be taking. This includes herbal supplements and over the counter medications as some of these can interfere with blood clotting and increase the risks associated with the procedure. The surgeon may ask that you cease using some medications for a time period before and after surgery to ensure the best possible outcome.

Blepharoplasty surgery is generally performed as an outpatient procedure. Recovery times vary though most people can return to normal, non-strenuous activities within a week. Again, an experienced surgeon can better inform you as to the expected outcomes and return to normal activity schedule.
